About A Golden Care Health Professional
A Golden Care Health Professional is a licensed or trained member of the Golden Care Solutions team who works under physician oversight to deliver coordinated, compliant, and patient-centered care through chronic care management and remote monitoring programs.

As a Golden Care health professional, you are an essential part of each patient’s primary care team. Your role is to educate, support, and advocate for patients while helping them better understand their health, manage chronic conditions, and achieve meaningful wellness goals.
Your primary responsibility is to listen to patients’ concerns, provide clear guidance, and work collaboratively with them and their providers to improve health outcomes, reduce unnecessary expenses, and enhance quality of life.
You typically connect with patients by phone on a weekly basis for approximately 10–15 minutes, while remaining flexible to spend additional time when needed to ensure patients receive the support and attention they deserve.
• Ensuring patients receive all appropriate and available healthcare services
• Coordinating communication, referrals, and documentation across the care team
• Maintaining accurate, up-to-date patient records
• Explaining care plans, treatment options, and next steps in clear, compassionate terms
• Addressing patient questions, concerns, and barriers to care
• Collaborating with patients on personalized care plans and health goals
• Providing disease-specific education and preventive health guidance
• Supporting continuous access to the care team, including after-hours resources
• Monitoring patient status to help reduce emergency room visits and hospitalizations
• Identifying opportunities to lower out-of-pocket healthcare expenses
• Facilitating access to remote patient monitoring, chronic care management, and related services
• Reviewing labs, screenings, and follow-up needs with patients
• Advocating for patients within the healthcare system
By consistently documenting patient interactions and sharing timely, accurate information with primary care providers, you enable physicians and clinicians to focus more fully on direct clinical care.
